ABSTRACT

OBJECTIVE: Voiding dysfunction may be occurred due to the herpes zoster with lumbosacral lesion. We studied the clinical symptoms in patients with voiding dysfunction caused by herpes zoster. METHODS: We have investigated the medical records in 10 patients. These patients had urinary symptoms associated with herpes zoster. RESULTS: Dermatome levels were sacral(s) in 10 patients (s2, 4 patients; s3, 6 patients). Urologic symptoms were acute urinary retention in 10 patients who showed detrusor areflexia (7 patients) or detrusor hyporeflexia (3 patients) in urodynamics. All patients were managed by inserting catheter with alpha blocker. After 1 week we removed indwelling catheter. All patients were no problem in urination. CONCLUSION: The cause of voiding dysfunction is not known clearly in herpes zoster infection. But we investigated that Famciclovir 750 mg using oral medication and the urethral catheter insertion performed by one week was recovered from herpes zoster. Authors think it is useful for patients with impaired urination.

ABSTRACT

PURPOSE: Nocturia is a bothersome symptom that impacts sleep quality in patients with lower urinary tract symptoms (LUTS)/benign prostatic hyperplasia (BPH). This study was performed to evaluate the impact of nocturia on sleep quality. MATERIALS AND METHODS: A total of 58 male patients with LUTS/BPH were enrolled. LUTS/BPH patients without nocturia were included in the control group. The inclusion criteria were eight or more points on the International Prostate System Score (IPSS) including more than one episode of nocturia and a prostate volume larger than 20 ml. IPSS, prostate volume, uroflowmetry, and the Pittsburgh Sleep Quality Index (PSQI) from each patient were recorded. RESULTS: Patients with nocturia showed a higher mean global PSQI (8.5+/-0.4) than patients without nocturia (4.82+/-0.4) (p<0.01). Patients with nocturia showed a higher percentage of severe sleep disorders (74.1%) than patients without nocturia (35.3%) (p<0.01). The regression coefficient between the number of episodes of nocturia and mean global PSQI was 0.42 (p<0.01). CONCLUSIONS: Patients with nocturia showed poor sleep quality, and this was related to the number of episodes of nocturia. This suggests that nocturia has a strong impact on sleep quality in patients with LUTS/BPH.

ABSTRACT

PURPOSE: Because of the volume of elective sterilizations performed in the world during the past decade, the vasectomy was a popular method for male sterilization in Korea and this, in turn, had been followed by an increase in the number of patient requiring vasectomy reversal with the high rate of subsequent divorce and remarriage. Recently, many authors have reported high success rates of vasovasostomy using microsurgical techniques and we performed modified two layer vasovasostomy with intravasal silicone tube to increase postoperative patency and pregnacy rate. METHODS: Microscopic vasovasostomy was performed in 9 patients at our department using modified two layer vasovasostomy with silicone tube insertion. Their ages ranged from 28 to 44 with an average of 35.78+/-1.36 years. Standard Guibor silicone tube, consisting of two 17.7cm, 0.064cm diameter, malleable, stainless steel probes connected by 29cm of silicone tubing wedged onto disposable probes, were used in all cases. RESULTS: Success rates were 88.8% for patency and pregnancy 44.4% for pregnancy in modified two layer vasovasostomy with silicone tube insertion. The patency rates were higher in cases of long postoperative day and in cases of short duration of vasectomy and vasovasostomy. CONCLUSION: We used a modified method to correspond the patency and pregnancy rate in microscopical modified two layer vasovasostomy using the intravasal silicone tube permanently. This method brings normal patency in microsurgical vasovasostomy because the silicone tube prevent obstruction of anastomosed site of the vas permanently.

ABSTRACT

PURPOSE: We evaluated the significance of the P504S expression in prostate cancer and also the usefulness of the P504S/34betaE12 combined immunostaining method for diagnosing prostate cancer, and we did this by performing histological analysis of needle biopsy specimens. MATERIALS AND METHODS: Prostate tissue specimens were obtained from 83 patients with clinically suspected prostate cancer. A total of 54 prostate needle biopsy specimens were immunostained with an enzyme commonly overexpressed in prostate cancer(P504S) and also with an antibody against a basal cell marker(34betaE12). A total of 83 cases were immunostained with 34betaE12, including 29 cases that were stained with only with HMW- CK(34betaE12). RESULTS: P504S immunostaining was positive in 96.3%(26 of 27 cases) of the prostate cancer specimens. 34betaE12 immunostaining was positive in 97.2%(35 of 36 cases) of the benign prostate tissues. Of the 30 P504S positive immunostaining cases, 26 cases were prostate cancers, 3 cases were ASAP and 1 case was ASAP+PIN. Of the 36 34betaE12 positive immunostained cases, 35 cases were benign and 1 case was ASAP. In the P504S(+)/34betaE12(-) cases, there are no benign prostate lesions. There are no benign prostate lesions in the P504S(-)/34betaE12(+) cases, and all the cases were benign. There were no statistical correlations between the grade of P504S staining and the clinical parameters such as serum PSA, the clinical stage and the Gleason scores. CONCLUSIONS: Combining P504S as a positive marker for prostate cancer with 34betaE12 as a negative marker might improve the diagnostic performance.
